The Patient Navigator is responsible for collecting, managing, and reporting required data related outcomes and measures. This requires both interpersonal and data management skills. The Patient Navigator assures that patients are connected with the right programs and services and seeks to close referral gaps that patients may encounter while receiving care.

Essential Functions

Achieves expected referral rate for internal and external programs; monitors assigned CMHC & CCBHC outcomes and coordinates with internal and external providers to increase frequency of desired outcomes. Utilizes outcome tracking and reports to help achieve successful outcomes and approve quality of care provided consistent with the practice and standards across the agency. Collaborates with other team members regularly with focus on improving quality of care.\ Collects and enters initial, follow-up, and discharge National Outcome Measures (NOMs) for clients as part of the CMHC & CCBHC outcomes.\ Builds effective, professional relationships by working in partnership with patients and families while emphasizing patient strengths, choice, and empowerment. Assessments and all patient interactions are delivered in a trauma-informed and respectful manner with integration of cultural competency across diverse populations.\ Achieves or exceeds hourly productivity standards as outlined by supervisor; achieves or exceeds target number of CMHC & CCBHC data collection requirements including outcome measures, NOMS assessments, 6-month follow ups, and discharges.\ Assures that key patient information is communicated to direct service providers and support staff prior to and after appointments. Confirms that patient has all required information to complete next steps in treatment prior to leaving the building after initial appointments. Facilitates collateral communication between patients, support staff, direct care providers, and the community to assist patients in meeting all health-related goals, with an emphasis on behavioral health.\ Positive working relationships with all resources within and outside the agency will be developed and maintained. Active and timely participation in all administrative and treatment staffing is consistent.\ CMHC & CCBHC data is entered into all required systems within all assigned timelines and requirements of the CCBHC model. Documentation is timely, current, and accurately reflects the services being provided and the patients response to those services.\ Patient assessments and other information is updated in a timely manner, and notes and other communications are sufficient to keep all team members informed of patient contacts, significant events, and follow-up steps that may be needed.\ Work Environment

This job operates in a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.
