This position is focused on helping connect the members we serve with housing navigation and support with the goal of helping them obtain and/or maintain safe, affordable housing. By addressing unstable housing, you will support UnitedHealthcare Health Plan of Nevada in our mission to build health equity and reduce health disparities leading to improved outcomes. You will solve complex problems as you serve as a resource to the member facing health plan teams and engage directly with identified high risk members experiencing housing related needs. Under guidance from the housing program manager, you will serve as a key member on a supportive, multi-disciplinary team that utilizes a range of evidence-based treatment modalities to help members work towards their housing goals. You will evaluate members social needs, identify appropriate resources, make referrals to community-based organizations, and establish and guide members through a personalized housing plan.

Primary Responsibilities

  • Provide office and community-based resource coordination and member education to help improve member wellness and quality of life

  • Provide Housing Supports and Services as defined in state contract to eligible members experiencing housing insecurity

  • Collaborate with internal teams and community organizations to support members through housing related needs

  • Assist members in managing daily obstacles, stresses and help support the development of life structure via work or other meaningful activities, addressing and developing life and illness management skills, and providing family education and support

  • Identify new and existing resource opportunities, through development of relationships with housing focused community organizations, property managers, local continuum of care and SDOH service providers

  • Coordination of services with other agencies, application and management of benefits, developing and implementing a housing treatment plan reflecting an outcome orientated and function-based philosophy

  • Serve as the liaison between the health plan and homeless assistance providers, including coalitions to end homelessness, local Housing and Urban Development (HUD) offices, Public Housing Authorities (PHAs), Continuums of Care (COCs) and community social services organizations

  • Develop and maintain local Housing Resource Guide for internal distribution and facilitates an ongoing forum to share information and best practices with the member-facing support teams

  • Coordinate with Social Drivers of Health team (e.g. employment, transportation, financial services, etc.) to facilitate access to SDOH support services, promoting successful tenancy and whole person care

  • Promote collaboration between UHC HPN Care Management and housing support providers to help members maintain both health and housing stability, including supporting escalated issue resolution through leveraging relationships and having a comprehensive understanding of eligibility requirements to advocate for members who experience barriers accessing community resources

  • Incorporate racial equity and diversity into all housing support and SDOH services and approaches. This includes, but is not limited to, monitoring and reporting on housing access for populations facing health disparities, supporting initiatives to achieve and maintain NCQA Health Equity Accreditation (and HEA PLUS) (if applicable), and participating in community solutioning discussions to support a more equitable housing/homeless services system

  • Responsible for regular reporting and documentation requirements

Required Qualifications:

  • High School Diploma/GED (or higher)

  • 3+ years of experience in healthcare or social services organization (i.e. behavioral health services, case management, addiction treatment, housing related services or social services agency)

  • Basic level of proficiency with Microsoft applications such as (Word, Excel, Outlook) and Internet

  • Access to reliable transportation to travel to work sites within a designated area

  • Able to work in the field and travel 50-75% of the time

  • Ability to work with medically and behaviorally complex members

Preferred Qualifications:

  • Bachelor’s degree or higher in healthcare, social work or related field. And/or Certified Community Health Worker or willingness to obtain certification within 90 days of employment

  • Familiarity with RENO/LAS VEGAS resource network including housing, medical, behavioral health, and social services

  • Demonstrated experience / additional training or certifications in evidenced based practices including Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care, and Critical Time Intervention

  • 2+ years’ experience in customer service / support

  • High level of interpersonal and conversational skills

  • Willingness and adaptable to changing people, process and technology

  • Attention to detail and follow up

  • Medicaid Managed Care Experience

  • Bilingual English/Spanish

  • Developing and implementing housing treatment plans

  • Good communication skills.

Nevada Residents Only: The hourly range for Nevada residents is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
