Posting Summary
Rutgers, The State University of New Jersey, is seeking a Social Worker for the Eric B. Chandler Health Center at the Robert Wood Johnson Medical School.Provides social work and behavioral health consultation utilizing a team-based model to children, adolescents, adults, and families in order to improve psychosocial functioning.Among the key duties of this position are the following:
  • Provides comprehensive assessment and diagnosis of behavioral health clients.
  • Provides social work support all health center patients.
  • Provides effective treatment planning and assisting clients in successfully achieving goals.
  • Evaluates crisis situations and apply appropriate interventions.
  • Actively participate in team and other meetings that support EBCHC's integrated health care model to provide comprehensive care for clients.
  • Assists in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration.
  • Assists the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.

Qualifications

Minimum Education and Experience
  • Master's Degree in Social Work plus two (2) years of post-Masters professional experience in a mental health field.
Certifications/Licenses
  • Must be a Licensed Clinical Social Worker (LCSW) in the State of New Jersey.
  • Valid driver's license.
Required Knowledge, Skills, and Abilities
  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.
  • Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communications skills.
  • Good knowledge of psycho-pharmacology.
  • Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
  • Knowledge of Word Processing Software.
Preferred Qualifications
  • Licensed Clinical Alcohol and Drug Counselor (LCADC) in the State of New Jersey.

Physical Demands and Work Environment
  • Physical Demands: Time spent on the physical activities: up to 2/3 standing, walking, sitting; over 2/3 using hand to finger, handle, or feel; over 2/3 reaching with hands and arms; no climbing or balancing; no stooping, kneeling, crouching or crawling; over 2/3 talking or hearing, and no tasting or smelling. Position requires up to 25 lbs. lifting. No special vision requirements.
  • Work Environment: A moderate noise.
