BayCare Health Systems is currently seeking a Supported Housing Advisor to join our outstanding and compassionate team!

Supported Housing Advisor Responsibilities:

• The Supported Housing Advisor assists, supports, and educates residents in activities of daily living to ensure functional independent living.

• Completes admission documentation, living skill assessments, treatment plans/updates, progress notes, activity notes, and discharge assessments.

• Ensures proper physical maintenance of housing, completes inspections, environmental rounds, and maintenance requests. Educates/monitors residents regarding appropriate use of medication.

• Completes HUD documents per HUD standards. Communicates with treatment team including case managers and HUD specialist regarding changes in condition and tenancy concerns.

• Completes incident reporting.

• Completes all documentation in accordance with policies/standards.

• Facilitates community meetings and acts as liaison between management and residents regarding meeting topics.

• Assists residents in managing emergencies, preparing for weather emergencies.

• Responds to emergencies according to program procedures.

Requirements:

• FL Driver's License (Coverage requires drivers to be 21 years old with 2 years driving experience (drivers over 69 may require a Driver’s Road test.)

• BLS (within 60 days)

• First Aide (within 60 days)

Education:

• High School Diploma - Required

• Associate's degree (Behavioral Health) - Preferred

Experience:

• 2 years - Health Care

• 1 Year of Behavioral Health - Preferred

Shift Hours: 8:00am - 4:30pm
