Job Summary:

In this role, you will engage with individuals with disabilities or those in recovery, assisting them in securing safe, affordable housing. Once their basic needs are met, you will provide support in other essential areas, actively resolving housing crises and serving as the expert on local housing resources. Collaborating with local Continuums of Care, you will identify those experiencing unstable housing and establish a network for supportive and stable living situations.

Key Responsibilities:

  • Practice Housing First, harm reduction, and low-barrier service models when working with individuals or households

  • Attend and participate in Coordinated Entry/Case Conferencing meetings within the local Continuum of Care

  • Outreach households experiencing housing crises or homelessness and not currently engaged in services

  • Refer and/or transport individuals to establish services for behavioral health, substance use, physical health, employment, etc.

  • Build and strengthen relationships with local landlords, providing education and referrals

  • Connect with referrals from Continuum of Care programs to assist with housing stability

  • Utilize SOAR training to assist individuals in applying for benefits

  • Assist households in identifying readiness for other permanent housing options

  • Participate in state-wide Housing Liaison training and calls

  • Complete timely data collection and entry as required

  • Become knowledgeable about all available community resources

  • Serve as a resource regarding housing and local/state resources

  • Assist eligible households in obtaining required identification documents

  • Assist with the moving on process and evaluate tenant readiness

  • Reengage participants who have become disconnected or at risk of homelessness

  • Help households identify and connect with natural resources to prevent or end homelessness

  • Understand HUD Quality Standards inspections

Education and/or Experience Qualifications:

  • Bachelor's degree in Social Work, Psychology, Sociology, or relevant case manager experience in housing; OR

  • Associate of Applied Science in Behavioral Health Support from an approved institution; OR

  • Qualified to provide Community Support based on criteria outlined by the Missouri Department of Mental Health; OR

  • Four years of qualifying experience in a related human services field, or a combination of education and experience equaling four years.

Required License/Certification (one of the following):

  • Mental health professional as defined in 9 CSR 10-7.140(2)(QQ)

  • Bachelor's degree in human services field

  • Four-year degree and two years qualifying experience

  • Four years of qualifying experience

  • Ability to develop new relationships within the community

  • Confidentiality in working with information

  • Proficiency in Microsoft Office applications

  • Basic math and problem-solving abilities

  • Crisis response and composure

  • Valid driver's license, acceptable driving record, and current auto insurance
